Preventing Identify Fraud
It is an NHS requirement that all new patients registering with the Practice produce proof of identity to verify the patient is who they claim to be and are entitled to NHS treatment. Each individual over the age of 18 will need to provide a minimum of two original documents supporting evidence of identity, and proof of residence in the practice area, which can include:
- Photo Driving Licence
- National Identity Card
- NHS Medical Card
- Utility bill
- Bank statement
- Council tax bill
When you visit the surgery for the first time you will be asked to sign the GMS1 form to confirm that the details are correct, and provide evidence of identity and proof of address.
When you register new patient Health Questionnaire and ethnic origin Questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
Online Medical Questionnaire For New Patients
Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice, together with proof of your identity.
Once you have sent the information to the practice, please present at reception during the following times:
Monday – Friday
11 am – 12.30 pm
3 pm – 6 pm
Please allow a minimum of 20 minutes to complete the registration process. Patients will be seen on a first come first serve basis and therefore you will be required to drop in and wait.
At this time, you will be issued with a username and password that will allow you access to the SystmOnline facility, through which you can book routine GP appointments and order repeat medication.
New Patient Appointments
Medical treatment is available from the date of registration. Please contact reception for further information.
All new patients who take repeat medication will need an introductory appointment with a GP to review medication and set up repeat prescriptions if appropriate.
In addition, all patients registering with the Practice can book a ‘New Patient Medical’ with a member of the nursing team. This will include taking measurement of your height, weight and blood pressure, testing a urine sample (specimen bottles available from Reception), and will also establish relevant past medical and family history, including medical, social and lifestyle factors. It is NOT possible to discuss medication at this appointment.
